Perio record card

Fits a ’25’ size pocket. The grids have spaces for recording pocket depths.

Enables the recording of six complete courses of treatment. Monitor the progress of disease and movements easily.

Complete the grids with the six measurements taken around the tooth. Mesially, at the mid point, and distally on both buccal and lingual surfaces.

Separate cards for upper and lower.

The grid allows for depths to be entered in millimeters.

On the reverse, along with the grids for BPE chartings, are some routine monitoring charts for bleeding and plaque indices.
